I hereby certify that I am physically fit and can medically participate in the #WearOrange Bridge Walk. On behalf of myself, I hereby waive and discharge DMV Mothers Like Me LLC, its sponsors, and affiliates from any liability or claims that may occur as a result of my participation, including but not limited to loss, damage, injury, or death.

I hereby agree, consent, and authorize DMV Mothers Like Me LLC to use my likeness in any photograph, video, or other digital media taken on Saturday, June 4, 2022, during the #WearOrange Bridge Walk in any and all publications, including but not limited to, print or web-based publications.
